An exciting opportunity has arisen for a self-motivated clinical pharmacist to work within GP practices in the Tywi/Taf Locality. The Tywi/Taf Locality covers the GP practices in Whitland, St Clears, Carmarthen Town, Nantgaredig, Llandeilo and Llandovery. These forward thinking GP practices are investing in this innovative role to deliver patient centred clinical pharmacy across the area. You will be responsible for providing prescribing advice to GPs and other health professionals and undertaking medication reviews in GP practices and care homes, with the aim of reducing risk, improving quality and promoting cost effective prescribing. You will liaise with GPs, nurses, practice staff, carers, patients and community pharmacists and work closely with the Health Board's prescribing support technicians.
You will have excellent communication skills with a demonstrated ability to work autonomously and collaboratively to ensure patients with complex pharmaceutical needs across the cluster are reviewed effectively, minimising the need for GP intervention. You will be highly motivated and influential with the ability to organise your workload to meet the demands of a number of GP practices.
The successful candidate will have a primary care, hospital or community pharmacy background and be passionate about developing the profession to support patients in a GP practice setting. This is a permanent post working 22.5 hours per week. As the post will involve travel to GP practices,you must be mobile to meet the requirements of this post.
The ability to speak Welsh is desirable for this post.
