Welwyn and Hatfield Locality-Community matron

We are changing the way we deliver our adult community services across east and north Hertfordshire to create a single, integrated model of care. Our aim is to make healthcare more preventative rather than reactive, and to empower local teams to tailor services to the needs of their local communities.

Commissioned by East and North Hertfordshire CCG, the service is provided by Hertfordshire Community NHS Trust, Hertfordshire Partnership University Foundation Trust and Hertfordshire County Council’s Adult Care Services. The partnership organisations are working collaboratively across nursing, therapy, mental health and social care to deliver better care for our population

An exciting opportunity has arisen in our new Community Integrated Care Team in Welwyn and Hatfield for a Community Matron, which delivers Prevention of Admission, Case Management and Core Nursing and Therapy.

The Community Matron will work within the Integrated team of Community Nurses, Therapists, CPNs and Social Workers to proactively manage, co-ordinate and support an identified caseload of patients with multiple complex long term conditions. You will have responsibility for ensuring all health and social care needs are met to improve health outcomes, functional status and quality of life. This will lead to increased patient choice and prevent unnecessary admissions to hospital and reduce the stay of necessary hospital admissions.

The post-holder will be an excellent communicator and work well both individually and within a team. You will be patient focused and have the ability to undertake clinical assessments to arrive at a differential diagnosis, start, plan and evaluate care. You will also have organisational and time management skills and be able to demonstrate leadership skills across the inter-professional team.

A full valid driving licence and access to a car to use regularly for business purposes is essential (unless you have a disability as defined by the Equality Act 2010).

In addition to the basic salary, this post attracts a High Cost Area Supplement of 5% of basic salary subject to a minimum payment of £1,017 and a maximum payment of £1,762 per annum (pro rata where applicable).
