As an Assistant Practitioner delivering NHS services we’ll provide you with access to a committed Learning and Development team, and we work closely with Universities and Health Education England to further your potential and support your progression. We will offer a combination of in-house eLearning, workshops and courses to support your development.
You will become part of Somerset County Council’s Public Health Nursing team supporting families as part of our 0-19 Healthy Child Programme.
As an AP you will visit Families and children in their own homes, as well as support well baby clinics in community venues and child health clinics in schools.
Your work with children and families will be to support the caseload holder as part of a holistic assessment. You will be allocated specific workload tasks by either the Health Visitor or School Nurse, and be responsible for reporting back to them as part of your ongoing work with families. This could be either short or long-term work. You will also be required to work closely with our Health and Social care colleagues; contributing towards multi-disciplinary work and assessments under the direction of the caseload holder.
You will have a childcare qualification and have experience of working with children and families 0-19, and be familiar with home visiting; previous experience working in the community is desirable.
You will need to be competent in the use of mobile devices as these are now used to record patient care.
