When people have had a major setback in life, it’s our Community Reablement Assistants at Hampshire County Council who support them regain their independence and regain control of their life.
After a hospital admission, service users may have lost their confidence or are unable to complete daily tasks as before. Perhaps they’ve had an operation or are recovering from a fall. Whatever their age, background or circumstance, our Reablement Service is designed to help service users regain the ability, independence and confidence to do some or all the things they used in a safe and realistic way.

The Reablement Team provide a full an in-depth supportive induction programme along with continuous training and development. There are opportunities for quick progression and fantastic prospects for career development. Some of our past Community Reablement Assistants have even gone on to becoming Occupational Therapists or Specialist Sensory workers.
It’s important to be able to travel between service users, but we can support you whilst you are working towards meeting the travel requirements, there is an opportunity for a car loan once you have joined us.

To work within the local area ‘Community Response’ Team. To deliver a short term social care reablement service in an enabling manner to complement a reablement programme with service users as prescribed by a Team Leader, or registered professional in Adult services e.g. occupational therapist. To promote a service user’s independence, choice and dignity. To support timely hospital discharge, hospital avoidance, crisis and or carer break down. To upskill and work with service users families or support network to enable, maintain and continue to supply support.Role Responsibilities:
