The Proactive Care Team is a new team that aims to develop and deliver a community based integrated model of healthcare for individuals with the most complex health needs. The team will work in the three Community Partnership areas of the Central Locality in Bradford.
Using a Lead Provider model, BDCFT will work in collaboration with key partners including Bradford Teaching Hospitals NHS Foundation Trust (BTHFT) as well as the GP Federations and Voluntary Sector partners to create a proactive culturally competent and aware Multi-Disciplinary Team (MDT). Team members will work to identify those with highest need and deliver a blend of preventative and recovery focussed interventions in the community, to support a reduction of premature mortality and help deliver improvements for those who are frail and those close to their end of life.
The overall aim of the Proactive Care Team will be to help reduce health inequalities for those with the most complex needs and support a reduction in Accident and Emergency Department (AED) attendances and unplanned admissions whilst also improving access to other existing community-based provision to help sustain good health outcomes.
We are looking to recruit a Band 7 speech and language therapist working within a new MDT model to provide assessment and intervention for swallowing and communication difficulties for patients with complex health needs who may also be frail and/or approaching end of life.
The Band 7 clinician will work alongside a Band 4 Clinical support Worker (CSW) who they will also have line management and development responsibilities for
The clinical role of the Band 7 speech and language therapist will include:
Reduce the risk of dysphagia (feeding and swallowing) and associated pneumonia
Advise around safe consistencies of diet and fluid in order to meet nutrition, hydration and medicinal needs
Advise around oral hygiene and saliva management
Promoting improved quality of life through safe eating and drinking practice
Improve transition from children’s to adult services –
Communicate EOL wishes and to support medical or treatment decision making
Providing interventions to improve communication skills to aide social inclusion, decision making and to improve health and wellbeing
Improving opportunities for access to work/ remaining in work
Provide support and treatment groups
Develop and provide resources (life story books, communication passports, communication boards and electronic devices.
Bespoke Swallowing Awareness Training to a range of health and social care professionals including; nursing/care home staff, chef and kitchen staff, care agency staff
Provide bespoke communication training packages to health and social care professionals
Able to provide resources that are culturally, linguistically appropriate
