CRG Homecare is looking for an enthusiastic, dedicated Reablement Officer to join our team in our Burnley Branch.

What you’ll be doing

You will be responsible for monitoring reablement customer goals, working closely with the Reablement Coordinator to gain feedback in respect to customer rehabilitation. Liaising with the Reablement Support Officers from the Local Authority to make recommendations and advise on customer rehabilitation. Completing 1st visit and initial assessments

What the role involves

This role would include collating feedback, from the Reablement Assistants on customer progression and making recommendations on the rehabilitation of Service Users by liaising with the Social Care Support Officers from the Local Authority.

Updating Reablement Assistants with changes that are made to Service User goals, visit times and visit pattern and monitoring Service User progression, along reporting concerns to the Local Authority.

You will also Liaise with Occupational Therapists and Physiotherapists to achieve best outcomes for Service Users rehabilitation and ensure they have the correct equipment in homes and to organise Service User visits to assess.

Ensuring all activities are fully compliant with Health & Safety policies and Procedures and will participate in new and existing Service User assessments and planning of Care and Support needs as appropriate.

You will be working on rotating work pattern which includes weekends and evenings and public holidays

Essential criteria

  • A minimum of 1 years’ experience within the social care industry (Essential)
  • Medication administration, care plan writing and accident reporting (Essential)
  • Caring and compassionate towards people in need of care and support
  • Respect for people suffering from a range of medical conditions with different backgrounds and beliefs to your own, commitment to non-discriminatory care practice
  • An excellent timekeeper and reliable

Knowledge and understanding

  • Good understanding of the needs of people who require care and support at home
  • Knowledge of effective care and support planning that is centred on the individual needs and wishes of each customer
  • QCF/NVQ L3 or equivalent or willingness to complete
  • Good understanding of health and safety matters in relation to home care services and of the risk assessment process
  • Sound understanding of the principles of good care practice

Experience and skills

  • Experience of communicating clearly and building positive working relationships with people who use social care services and social and healthcare professionals
  • Ability to support customers with all aspects of their daily living in a manner that respects their dignity is non-judgmental and promotes their independence, choices and privacy
  • Experience of care services
  • Skills in care and risk assessments and writing care and support plans
  • Ability to plan and organise workloads effectively so that customers receive the services that they expect
  • Ability to use own initiative and work alone or as part of a team especially in an emergency
  • Good administrative skills and computer literacy
  • Ability to lead and manage a team of care workers and provide appropriate support, guidance and training
  • Ability to maintain clear written records and follow statutory reporting procedures
