Nutrition plays a vital role in supporting my long-term conditions of primary progressive multiple sclerosis and a bipolar disorder. I tend to prefer fast foods or ready meals to fresh cooked food and need support to make more appropriate choices in terms of daily food and drink.
I am a permanent wheelchair user, not fully weight-bearing but continue to transfer independently with close supervision and some assistance with positioning the wheelchair, as required. Help is required to prepare for and access the wet room as my ability and safety varies unpredictably, linked to symptoms of MS. Some personal care is carried out independently, with minimal assistance.
Support and assistance is required to take prescribed medication is essential and may include as-required pain relief. It is important to monitor and report their effectiveness and potential side-effects, for stability and improved quality of life.
Personal choice and being listened to are very important to me. I have difficulty organising my personal possessions and to keep my home environment clean and tidy. Assistance is required to support independence in my home, for both physical and psychological wellbeing.
