Urgent Response Service (URS) is an integrated Health and Social care team, involving Southampton City Council and Solent NHS. We work together to facilitate early discharges from acute settings and prevent hospital admissions. We support adults in their own homes across Southampton to maximise their independence with the focus on reablement to minimise their need for ongoing care.
We are looking for an experienced, confident and self-motivated individual to undertake the role of Social Worker within the Urgent Response Social care team. There will be an emphasis of employing a strengths-based approach when carrying out assessments. You will need to be able to deal with these efficiently and effectively as Urgent Response is a fast-paced team.
You will be responsible for advising a small team of care managers and manage a range of cases where you will need to risk assess and put protection plans in place to safeguard our service users. You will also be required to process referrals made for ongoing care packages. The Urgent Response Team is a high achieving team that has been meeting its targets towards keeping its service users safe and independent within their own homes.
It is essential for you to be able to work flexibly both in the office and out in the community and be able to work with multiple agencies, clients and their families when under pressure and managing risk.
