Hours: 30 hours
Pay: £12.00 - £13.00p/hr Ltd
Contract: 12 Weeks+
Responsibilities: The candidate will carry an active caseload providing brief interventions and psychosocial interventions supporting service users from their point of entry into the service and throughout their treatment and recovery.
Daily duties will involve delivery of integrated service through assessments, recovery planning, risk planning and management, key working sessions and all other functions associated with intensive case management and care coordination. The applicant will carry out triage and assessments and assist in the referral of individuals between specialist agencies. The applicant will support individuals to undertake and monitor their own health care, to support reintegration and enabling them to lead a meaning and purposeful life: promoting recovery, resilience and independence.
Person Specification: The successful candidate must have 1 years’ experience on their CV of working within a substance misuse specific role.
