- Provide high quality care and support to vulnerable individuals.
- Provide supervision to a caseload of Care Support Workers.
- Be proactive to ensure effective preparation of Care Plans and Risk Assessments, for approval by the Registered Manager.
- Undertake Care Support Worker supervisions and quality assurance checks.
- Undertake appraisals for Care Support Workers.
- Create and manage individual care delivery files, ensuring that care plans are kept up to date and records are kept as required.
- Ensure that all relevant stakeholders receive a copy of individual care plans in line with data protection guidelines.
- Liaise with all relevant stakeholders, i.e. Service-users, families, social workers, district nurses etc.
- Undertake any administrative duties associated with the role when present in the office.
- Ensure all service user / staff files are kept updated.
- Undertake service-user reviews in clients’ homes at regular intervals as agreed with the Registered Manager (usually on a 6 monthly basis).
- Ensure that all relevant information is input and stored on relevant software.
- Take care of any issues arising with staff and/or service-users, ensuring that files and key stakeholders e.g. Social workers, District Nurses are kept updated.
- Any other duties which may be assigned from time to time in line with the responsibilities and duties of the post.
