Band 5 Community Nurse

WHAT DOES THIS ROLE INVOLVE:
·Community Nurse Registered with the Nursing and Midwifery Council (NMC)
·Undertake assessments in order to implement and evaluate programmes of care for patients in the community.
·Patients include those with complex needs and long term conditions.
·Support palliative patients to remain in their chosen place of care
·Refer to the appropriate allied health care professionals as required.
·Proactively promote healthy lifestyle choices. Recognise situations that may be detrimental to health referring to the appropriate agencies as required.
·Operate within trust policies and guidelines.
·Document accurate records of care. Maintain up to date paper and electronic notes.
·Maintain confidential patient information to comply with Information Governance policy.
·Delegate to junior team members ensuring tasks are within health and safety regulations.
·Encourage patients and their families to be involved in planning, delivery and evaluation of their care through service user groups and the PALs service.
·Work within the multi-disciplinary team to co-ordinate and manage patient care.
·Support and mentor pre-registration students
·Work independently with minimal supervision
Nursing certificate and registration with the NMC with a valid UK PIN number
·You will require current NMC registration as a Registered Nurse.
·An ability to use a laptop to record and access patient information.
·Have worked as a nurse with ideally 2 or more years' experience.
·Ability to drive, with current full driving licence
